Exhibit 2 shows the Data Input Form. You will note that you can enter either the month, day, and age (if known) or actual birthdate and the program with calculate the other. If, as is often the case, you only know the month and day, the program will assume the year to be the current one and give no age. There is an information field for any special data if desired - suce as "graduated Lakeside High." The City field fills in automatically with the default value provided by the user, but can be changed at data entry.
